Thrombotic thrombocytopenic purpura associated with cerebral SLE.

چکیده
A case of thrombotic thrombocytopenic purpura in a patient with a recent flare of cerebral lupus is presented. The patient presented with neurologic symptoms, pyrexia, thrombocytopenia and leucocytosis. Following initial investigation, imaging of brain and negative microbiology, a diagnosis of cerebral lupus was made and appropriately treated. After a stable period oftwo weeks, deterioration in clinical status, haematology and renal indices, a diagnosis of thrombotic thrombocytopenic purpura (TTP) was made and treated. The two diagnoses presenting together are briefly discussed.

Thrombotic thrombocytopenic purpura preceding systemic lupus erythematosus.
The case of a patient admitted with thrombotic thrombocytopenic purpura nine years after developing systemic lupus erythematosus (SLE) is reported. Thrombotic thrombocytopenic purpura associated with SLE has been described on other occasions, but in most patients the diagnosis of SLE precedes that of thrombotic thrombocytopenic purpura. The unusual sequence and the chronological separation of t...
متن کاملSuccessful Treatment of Refractory Thrombotic Thrombocytopenic Purpura with Rituximab, a Monoclonal Antibody: a Case Report
Thrombotic thrombocytopenic purpura (TTP) is mostly attributed to the presence of an autoantibody against ADAMTS-13, a metalloprotease that degrades ultralarge von Willebrand protein multimers. Accumulation of vWF multimers and systemic platelet aggregation lead to microangiopathic thrombosis, hemolytic anemia, and end-organ ischemia. Most patients respond to therapeutic plasma exchange (TPE), ...
متن کاملThrombotic thrombocytopenic purpura and deep vein thrombosis as the presenting manifestations of systemic lupus erythematosus: A case report and review of literature
Systemic lupus erythematosus (SLE), is sometimes complicated by the rare fatal syndrome, Thrombotic thrombocytopenic purpura (TTP), but the occurrence of TTP as the initial manifestation of SLE is very rare. Since they have similarities in some features, the differentiation of TTP from SLE may be missed. SLE patients are also more prone to thrombotic events. Here we report a case with TTP and d...
متن کاملSystemic lupus erythematosus and thrombotic thrombocytopenia purpura: a refractory case without lupus activity.
The association between systemic lupus erythematosus (SLE) and thrombotic thrombocytopenic purpura (TTP) has been infrequently reported. Usually, patients with TTP have more SLE activity and frequent renal involvement. Here we present a case of TTP associated to low-activity SLE. The absence of renal and major organ involvement increased the difficulty in making the initial diagnosis. ADAMTS13 ...
متن کاملA patient with SLE-associated thrombotic microangiopathy and non-neutralizing antibodies against ADAMTS13.
In this case report, we describe for the first time a patient with thrombotic thrombocytopaenic purpura (TTP) accompanying highly active systemic lupus erythematosus (SLE) that was associated with non-neutralizing antibodies against the plasma metalloprotease ADAMTS13. Those non-neutralizing antibodies could have been the cause of ADAMTS13 depletion and consecutive TTP. More extensive analyses ...
